Designed to connect local people with community physical activity clubs, groups and facilities, you can get advice on how to move more and discover what’s going on in your local area!

What we offer

  • Support and advice from a qualified exercise specialist about what physical activity options might suit you best.  
  • Preferential rates to access some of our partner clubs.  

Surgery benefits

  • Helps the surgery achieve QOF outcomes associated with smoking cessation

  • Paid by outcome model means your surgery could receive up to a maximum of £80 per patient that quits

  • Reimbursement of Nicotine replacement therapy (NRT) or any prescribed Champix/Zyban on a quarterly basis

  • Direct contact with patients, removing the barrier of a patient not signing up to our service on referral (strike whilst the iron is hot!)

  • Over 18’s. PLUS
  • Those doing less than 150 mins of physical activity a week.
