Designed to give over 55’s a tailored, holistic service to improve overall health. Improve your physical activity levels, mobility and learn more about healthy aging with info on nutrition, mental health, falls and dementia prevention.

What we offer

  • Support from a qualified Health Coach.  
  • Weekly physical activity sessions.  
  • A useful booklet containing lots of information and advice on healthy aging.

Surgery benefits

  • Helps the surgery achieve QOF outcomes associated with smoking cessation

  • Paid by outcome model means your surgery could receive up to a maximum of £80 per patient that quits

  • Reimbursement of Nicotine replacement therapy (NRT) or any prescribed Champix/Zyban on a quarterly basis

  • Direct contact with patients, removing the barrier of a patient not signing up to our service on referral (strike whilst the iron is hot!)

  • Over 55’s. PLUS
  • Those doing less than 150 mins of moderate physical activity a week.
