We’ve teamed up with Slimming World to offer eligible individuals a funded 12-week voucher to access their local face-to-face Slimming World group.

- See real results – reach and maintain the weight and size you want to be, in a way that lets you live your life, and feel confident that you can stay there for good.

Please note:
One You Lincolnshire cannot provide a funded Slimming World voucher if you have been a paying Slimming World member within the last 3 months.
Only one funded voucher can be issued within a 12-month period.
Your Slimming World membership includes up to two weeks for illness or holidays, which can be used at any time during your programme. Any additional missed weeks beyond this cannot be guaranteed to be added to your membership. If you are unable to attend your Slimming World group for more than two weeks, please contact Slimming World on 01773 302487 or inform your consultant as soon as possible.
Surgery benefits
Helps the surgery achieve QOF outcomes associated with smoking cessation
Paid by outcome model means your surgery could receive up to a maximum of £80 per patient that quits
Reimbursement of Nicotine replacement therapy (NRT) or any prescribed Champix/Zyban on a quarterly basis
Direct contact with patients, removing the barrier of a patient not signing up to our service on referral (strike whilst the iron is hot!)
